FOR MINISTRY OF HEALTH, SAUDI ARABIA SELECTED DOCTORS
MAR  2010  SELECTION

  1. The doctors, recently selected by Ministry of Health, Saudi Arabia team are informed that they are required to travel to Saudi Arabia within 35 days from receipt of visa letter from this office. Details of selectees and the documentary formalities required to be completed are tabulated in sequence on our web page. The doctors are requested to complete the documentation process (including document attestation from respective boards, IBCC, HEC, Foreign office and Ministry of Health) and deposit these in our office by 06 Apr 10. Further documentary process with Saudi Cultural Office and Consulate is as follows: -

PARTICULARS OF THE DOCTORS SELECTED FOR EMPLOYMENT WITH MOH, SAUDI ARABIA, MAR 2010 BUT TO WHOM JOB NUMBERS NOT YET ALLOCATED

a.       Karachi Candidates Only

  1. Will deposit their degrees for attestation in Saudi Cultural Office, Karachi with Mr Haroon alongwith following documents. These documents will be attested from Saudi Cultural Office, Islamabad: -
  2.       
    a) Original degrees
    b) Photo copy of selection letter
    c) Photo copy of salary contract letter
    d) Photo copy of passport
    e) Photo copy of ID Card
            

  3. After attestation from Islamabad Cultural Office, these documents will be sent to Karachi Consulate for collection by the candidates. Then the complete documents will be sent to this office for visa issuance by Saudi Consulate, Islamabad.

 b.      All Remaining Selected Candidates.

Will deposit their complete documents with Fauji Foundation (Overseas Employment Services, OES) Rawalpindi for attestation from Saudi Cultural Office and Consulate, Islamabad.

2. Candidates are once again requested to complete their documents in time.   

3. Your inability to deposit documents in time / refusal to proceed to Saudi Arabia may black-list you by the MOH for future employment.

Passport

  1. Original machine readable + two photo copies
  2. Old passport(s) are also required to be attached with the existing passport.
  3. Minimum validity period of passport required is two years.

2.

Medical Fitness Report

  1. Medical examination will be done through GAMCA. We will refer you to a particular medical centre.
  2. Original passport will be produced at the time of medical examination.
  3. Following documents will be submitted to OES:
    1. Original Medical Report + two photocopies.
    2. For family visa. Medical report + two photocopies of wife & children above 12 years. Also Polio Vaccination Cards up-dated and issued from authorized GAMCA medical center.

Note:

Following documents will be required after obtaining Medical Fitness Certificate.

3.

N.I.C (issued by NADRA)

Original + two photocopies.

4.

Educational Degrees / Certificates

Following documents will be submitted in OES office (in original with Arabic translation)

  1. Secondary and Higher Secondary School Certificates. Duly attested by concerned board, IBCC and Foreign Office (Original + two photocopies).
  2. PMDC / Diploma. All attested by concerned Ministry of Health, HEC and Foreign Office (Original + two photocopies).
  3. FCPS / MCPS / MBBS / BDS. All attested by HEC, Ministry of Health and Foreign Office (Original + two photocopies).

Experience Certificates

  1. Only experience certificates of government departments will be translated in Arabic and attested by Ministry of Health and Foreign Office (Original + two photocopies).
  2. After obtaining MBBS Degree, the experience certificates should be chronologically updated without any break/gap.
  3. Experience certificates of private hospitals/organizations are not to be translated in Arabic.

NOC /Affidavit 
(for selected female candidates)

All lady doctors to provide affidavit on Rupees 20/- stamped paper (married from her husband and un-married from her parents) stating that he / she has no objection to the lady’s proceeding abroad (Original + two photocopies).

7.

Nikah Nama

Original + Arabic translation to be attested by Ministry of Foreign Affairs.

8.

NADRA Form “B”
(Registration Form)

Original + Arabic translation to be attested from Ministry of Foreign Affairs (Original + one photocopy).

9.

Photographs with Sky Blue background (self , wife and children-in case of family visa)

Passport size (4 x 6 cm)   =  08 (per individual)

10.

Affidavit.

  1. Candidates serving in government / semi government departments and retired Armed Forces personnel will provide departmental ex-Pak Leave / NOC from parental organization.
  2. All others will produce an Affidavit on judicial stamped paper of Rs. 20/- stating that they are not Government/Semi Government employees.

Visa Form

To be returned to OES duly filled in.
